Job summary This will be a split role partly as a Care Coordinator whilst also being trained as a Receptionist to provide cover when situation requires it. As a GP Receptionist/Care Coordinator you will be one of the first points of contact with the Practice for our patients. Our reception staff are a close knit team who take on the challenges of dealing with people who are not always at their best and providing them with the best service possible in the circumstances. This may be either on the telephone, face to face or via messages through our Practice website. We are therefore seeking a Team player and someone who can communicate easily with staff and patients alike. The job will be split between working as a part of the Reception Team and also working independently as a Care Coordinator. Main duties of the job Care Coordinator: Work closely with GPs and practice teams to support them to manage a caseload of patients to develop individual personalised care and support plans, ensuring appropriate support is made available to patients and carers, helping them to understand and manage their condition and ensure changing needs are addressed. Review patients needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate Assist GPs with referrals, insurance reports and other administrative duties. Liaise with Social Prescribing Link Workers and Complex Care at Home on current health resource and future work Liaise and work with the ICB, practices, Integrated Locality Team, voluntary and other organisations to implement patient services. Reception: Deal with all patient enquiries and navigate them in the most appropriate direction to assist them with their needs, booking appointments where required with the relevant clinician. Good communication skills are essential both face to face and by telephone. You will be expected to assist all clinical staff with appointment booking and handling of paperwork relating to patient referrals, test results and reports.
