Due to the implementation of a new Macmillan Information and Support Service across the Trust, we are wanting to recruit motivated care coordinators to join the team.
This is a 24 month fixed term/secondment opportunity and we have funding for 3.4wte posts, either full or part time working.
The posts will work cross site in providing an information, listening and sign posting to cancer patients and their families for support. This is part of the Trusts agenda to improve support, health and wellbeing and reduce inequalities. Part of this role will involve phone contact in providing a responsive agile service as well as face to face interactions. The post holders will work with existing teams in primary and secondary care in supporting patients across all sites.
You will work with existing cancer teams, primary and secondary care colleagues and voluntary sector in providing support and sign posting to services.
The post holders will be responsible for supporting patients and their families through cancer pathways and living with an beyond a cancer diagnosis in addition to interface with community and primary care services as required by the patient’s individual needs. They will work under the direct and indirect supervision of the personalised care lead /manager of the service and be responsible for coordinating care for patients with complex care needs and providing a point of access to patients and carers.
We manage three major locality hospitals at North Tyneside, Wansbeck and Hexham, plus a number of smaller community hospitals and clinics from Tynemouth to Berwick on Tweed, in addition to our state-of-the-art Northumbria Specialist Emergency Care Hospital, the first of its kind in England. We also care for people in their homes and provide services from facilities in local communities such as health centres. We give people greater choice and control over their care to help them to live independently at home and avoid hospital admission where appropriate. The overall aim of these posts is to facilitate the support and sign posting to current services eg benefits advice, local support groups.
The development of the role will be led by the needs of the service and the successful candidates therefore will be adaptable and willing to work flexibly with individual personal development encouraged and supported.
The posts will require cross site working and also link in with community projects so it is essential that the post holders have a current driving licence and can offer and meet the flexible service requirements.
