Job Title: Senior Staff Nurse Band 6
Responsible To: Integrated Nursing Service Lead
Accountable To: Director of Clinical Services
Pay Scale: Agenda for Change Unsocial hours payment payable dependant on shift pattern
Hours of work: Full time / part time considered. Flexible working. Internal rotation between day and night shifts (11.5 shifts).
Job Summary
To work as part of the Integrated Care Team providing symptom management, palliative and end-of-life care, as well as planned and emergency short breaks both at the hospice, in the home or hospital. The postholder will take on a lead role to support work required to ensure high standards of care are delivered and maintained. The post holder will be required to work a variety of shifts as the hospice provides care services 24 hours a day, 365 days a year.
Core Responsibilities
Clinical
1. To support the team with the implementation of high-quality services including delivering direct care to babies, children, young people and their families, embedding effective and supportive leadership.
2. To co-ordinate the shift/caseload work, ensuring safe, effective and responsive delivery of individualised care.
3. To provide high quality nursing care for children and young people who have palliative care needs in the hospice and community as required as part of a child led integrated service.
4. To contribute to the care plans and assessments required to ensure the child/young person receives safe and effective care.
5. To be fully conversant with and contribute to all clinical guidelines, policies and procedures of Haven House.
6. To communicate complex and sensitive information effectively to service users, care staff and other members of the team.
7. To work with the education team and further support junior colleagues with accountability for health care support workers.
8. To ensure that safeguarding is embedded in practice and concerns are escalated at the earliest opportunity to the safeguarding lead.
9. To contribute to effective medicines management in accordance with Haven House policies.
10. To effectively use the electronic patient record system to ensure accurate and timely data is entered.
11. To demonstrate a professional approach to work and act in accordance the NMC code.
12. To be available to partake in line management of junior staff working towards effective development, wellbeing and progression for each team member.
13. To be part of the senior nurse on call team, rotating to covering on call duties for both planned, unplanned and end of life care.
Work Collaboratively with Other Team Members
1. To contribute to the evaluation of the service as required ensuring that children, young people and family's views are represented.
2. To work closely in collaborative relationships with multidisciplinary teams, such as allied health professionals or external stakeholders like schools and social care teams.
3. To be aware of allocated resources and ensure effective time management in own role.
4. To promote good relationships and good working practice across teams.
Clinical Governance
1. To participate in audit, service user feedback collection and research as required.
2. Responsibility in monitoring and maintaining compliance with CQC (Care Quality Commission) standards.
3. To report all clinical and non-clinical incidents and accidents and engage in incident reviews as a learning opportunity in order to improve service.
4. To constructively identify issues with care strategy, policies and procedures in the interests of the organisation and users of the service.
Educational
1. To identify own learning needs and ensure professional development is maintained.
2. To engage in Haven House performance and appraisal system.
3. To contribute to overall staff and service development by updating the evidence base for the team re clinical practice.
4. Education and training of parents/carers to meet the health care needs of their child.
5. Education and training of junior staff and students as required.
Other Duties
1. To promote and live the Haven House values of Compassionate, Professional, Respectful and Safe.
2. To work with colleagues across the hospice as part of the Haven House "One Team approach" fostering collaboration across all care disciplines, ensuring integrated, child-centred care.
3. To conduct yourself in line with the Haven House cultures and behaviours at all times, acting as an ambassador for the hospice.
4. To actively participate in continuous improvement initiatives to support achieving an Outstanding rating.
5. To offer flexible support for the work of the organisation, with a flexible approach to new duties and responsibilities as needed to support our vision, mission and values.
