As a Community Matron you will be responsible for leading and managing the Southall Locality Team with the support of your Team manager, to ensure that all patient’s needs are met through the delivery of high-quality care. As a centre of excellence, your team will offer a holistic, person-centred approach promoting/ enabling the patient, family and carers to effectively manage their care. You will also work within a Network of statutory and 3rdsector providers and services, supporting our patients with nursing needs as required. In addition, each of the Locality teams have designated Care coordinators who work alongside the team in support of individuals with complex care.
The post holder will be responsible for all aspects of the clinical and management support to their team, ensuring that staff are equipped to undertake high quality care and developed within their role. In addition, the post holder will be accountable for all aspects of quality, resources and performance within their team.
The successful post holder will be provided with regular support and supervision and development opportunities
• Maintain a high standard of clinical work and compliance within the teams.
• Ensure compliance with our clinical governance framework, learning from incidents, you will motivate colleagues, champion service user safety and dignity and drive continuous improvement.
• Lead as part of the locality community nursing team, working collaboratively with the wider multi-disciplinary locality team and acute services.
• Provide leadership on the care and management of patients with complex needs, improving care, and preventing unnecessary hospital admission, support timely and safe discharge and establish effective personalised care and treatment plans to support people to remain in their usual residence.
• Provide education and support to individuals and their families to promote self-care strategies and independence.
• Work with social care and other community services once self-care is optimised and admission risks reduced and/or another professional has taken on the care co-ordination role.
• To attend weekly meetings with a multi-disciplinary team to review patients on the caseload, prioritising admission avoidance and facilitation of discharge.
• Support the team manager in the development and deployment of a skilled, competent and motivated workforce, able to meet the needs of the patients on the locality team caseload.
• To facilitate, motivate and supervise members of your team, including students under your management.
