Focus on safe/effective discharge planning reducing the risk of readmission. Main duties of the job We are looking to recruit a highly motivated, enthusiastic staff nurse to work as part of our multidisciplinary team. Fixed term to May 2026 for maternity cover, it is a part time role (36 hours) across Monday to Sunday. Bank holidays and weekends included. The Jean Heyes Reablement Unit is run by a team of dedicated specialist community practitioners who are experts in providing the most appropriate care for our frail & elderly population. Applicants will be required to see patients and families face to face within a community setting. An Enhanced DBS with Adults' Barred List check will be obtained during pre-employment checks.
