Job summary This post will provide GP clinical pharmacy services to Cauldwell Medical Centre, and can be onsite or hybrid. The Primary Care directorate at ELFT have an exciting opportunity for an enthusiastic, motivated, and innovative Senior Clinical Pharmacist to join our Team. We are looking for an individual who will support medicines management and be a clinical leader for medicines optimisation within Cauldwell Medical Centre. Main duties of the job Work as part of a multi-disciplinary team taking responsibility for areas of chronic disease management within the practice Undertake clinical medication reviews to proactively manage patients with complex poly-pharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities Provide primary support to general practice and GPSU staff with regards to prescription and medication queries Clinical leadership of medicines optimisation, plan and organise quality improvement initiatives, appropriate specialist services and manage some aspects of the quality and outcomes framework and enhanced services The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
