As a registered nurse and midwife, work within the relevant scope of practice to deliver care in accordance with Nursing & Midwifery Council standards (or other recognised regulatory body), & locally adopted guidelines & policies. The post-holder will be expected to utilise their professional skill and judgement to appropriately and safely manage their case load within the resources available, in conjunction with the multi-disciplinary team. Undertake direct patient care demonstrating competence, acting as a role model & ensuring any alteration in health is noted, care plans and medical records updated and deterioration recognised and acted upon in an appropriate and timely manner. This to include patients on the general ward with any presenting condition, and whilst working as a midwife this may involve delivering care at any time during the ante, intra, and postnatal period. Deliver high quality, evidence-based individual models of care which comply with locally-adopted guidelines & policies. Reviewing & updating local guidelines & policies in respect of midwifery care via established procedures for doing so where required to ensure currency with internationally-recognised best practice. Promote health and optimise outcomes by providing education to patients and families on a one to one basis or where appropriate through group work. Liaise with the multidisciplinary teams & outside agencies about identified risks including acting on advice from overseas consultants or contributing to referral overseas in a planned, urgent or emergency situation. Support patients, their families and the public equally with dignity and respect recognising diversity and demonstrating equality of opportunity with a rehabilitative approach or supporting a peaceful death. As a registered nurse, giving care to patients in the acute setting, providing care to patients with a diverse range of presenting conditions and backgrounds. Identify and manage risk in the context of a remote environment, plan care & treatment pathways, and ensure the best possible outcomes for both planned and emergency episodes of care. As a midwife, promote the health of women with the provision of pre-conceptual care to improve the outcome of pregnancies in regular clinics as well as with community outreach. Act as patient advocate in promoting and delivering the highest standard of nursing midwifery care in accordance with recognised best practice, ensuring all fundamentals of care are addressed, and current, relevant policies, guidelines and procedures are adhered to. Adherence to, and compliance with the Falkland Islands Government & Department of Health & Social Services Information Technology & data management policies & procedures, including contributing to the housekeeping & accurate updating of computer-based patient records. Provide collaborative antenatal care in the community and/or hospital to identify and manage risk; enabling informed choices to plan and provide care whilst appropriately adapting to the needs and circumstances of the pregnant women with our community. Provide intrapartum care to women throughout their labour and delivery including in planned, urgent or emergency situations together with referring & consulting with the wider multi-disciplinary team. Provide postnatal care to women and their baby within the hospital and community setting to enabling the women and families to make informed choices within a safe environment to facilitate the adaptation to parenthood and achieving the familys optimum independence. Ensure that a culture of continuous improvement in the provision of nursing and midwifery care is promoted within the clinical governance structure & undertaking training/CPD in order to meet professional registration requirements amongst other tasks.
