Job Title: Social Worker – St Helens – Older People

Liquid Personnel is currently recruiting for an exciting role. We are looking for a Social Worker to join a busy Older People Team based in St Helens.
What will your responsibilities be?
You will assess and manage the complex needs of older people and their carers to ensure their health, wellbeing, and social inclusion. This involves liaising with Reablement and other professionals to ensure seamless care pathways that safeguard and promote independence. You will also promote partnership working to support the delivery of person-centered outcomes for older people. Additionally, you will undertake safeguarding investigations as required.

Qualifications and Experiences:
You must have Social Work England registration, be eligible to work in the UK, and hold a full UK driving licence. Post-qualified experience and a degree level or equivalent qualification in Social Work are also required.
