The Pharmacy Clinical Coordinator is responsible for coordinating clinical pharmacy services and ensuring that the pharmacy provides optimal clinical services on a daily basis. The Pharmacy Clinical Coordinator facilitates the development, implementations, and maintenance of systems that promote desirable patient outcomes through the use of safe, cost-effective drug therapies.
Responsibilities:
* Facilitates the development and implementation of treatment guidelines, protocols, order sets and formulary changes as approved by the CNE P&T Committee. Collaborates with fellow Pharmacy Clinical Coordinators throughout the system to align with implementations.
* Tracks, reports, investigates and implements changes related to medication safety events including adverse drug reactions (ADRs).
* Ensures compliance with medication management, quality and safety standards/regulations set forth by the Joint Commission, Centers for Medicare/Medicaid Services, and other regulatory bodies.
* Develops, organizes, and evaluates clinical activities for Clinical Pharmacists.
* Tracks pharmacy intervention metrics for Clinical Pharmacists.
* Participates and supports inventory coordination with direct oversight and management of supply shortages. Reviews drug utilization for high risk and high-cost medications.
* Develops and provides education to providers, pharmacists, hospital staff and learners to optimize safe and effective medication use.
* Represents Pharmacy Department on assigned committees with a focus of assuring safe and effective use of medications. Oversees and Chairs/Co-Chairs CNE P&T Committee, Medication Safety Management Committee, and corresponding meetings.
* Communicates new information from the CNE P&T Committee to medical, pharmacy, and nursing staff.
* Coordinates pharmacy student learning experiences for the institution.
* Performs all other duties as assigned.
Precepting Pharmacy Residents
In addition to the above responsibilities, the Pharmacy Clinical Coordinator participates in the PGY1 pharmacy residency program as a preceptor for at least one required Administration rotation and the Longitudinal Clinical Administration rotation for each resident and may precept additional elective rotations and/or the major project. The pharmacist will model, coach and facilitate the resident’s learning to be able to perform the duties of Pharmacy Clinical Coordinator.
Job Qualifications and Specifications
Doctor of Pharmacy (PharmD) degree with PGY-1 post-graduate residency training plus two years of experience in clinical pharmacy (or a minimum of 5 years clinical pharmacy practice experience without a residency) is required. Valid license to practice pharmacy in Rhode Island is required. Board Certification in Pharmacotherapy (or Board-eligible) is preferred but not required.
