This is an exciting opportunity for a highly motivated, enthusiastic and experienced nurse to join our established heart failure service.
In partnership with cardiology consultants and the hospital heart function team, you will be expected to provide high quality, individualised, evidence based care to patients with heart failure and their families. In addition you will act as a role model for others, supporting other healthcare professionals both in secondary and primary care, to manage the complex needs of this patient group.
The post holder should have excellent communication skills and the ability to work independently as well as part of a multidisciplinary team. It is also expected that the candidate will be a first level registered nurse with current NMC registration and hold a post registration qualification in cardiology with significant cardiology experience. Non-medical prescribing qualification or willingness to study for this is essential.
Clinical Practice
1. Demonstrate expertise within the field of HF whilst identifying and assessing patients admitted with a diagnosis of heart failure, including expediting echocardiography to confirm diagnosis where necessary.
2. Ensure that all patient contact is documented and captured for submission to NICOR database.
3. Provide written and verbal education and advice to patients with HF.
4. Promote a culture of nurse-led decision making within clinical areas through the implementation of evidence based guidelines.
5. Work in close collaboration with the palliative care team in hospital, raising awareness of the need for palliation of symptoms with colleagues in secondary care.
6. Deal professionally and sensitively with patients and their families when discussing diagnosis, implications for future health and regarding prognosis and end of life issues.
7. Exercise professional accountability at all times and be aware of own development needs/limitations, actively seeking to address these.
8. Provide specialist advice and support to nurses and members of the multidisciplinary team and act as a role model.
9. Arrange appropriate follow-up of patients admitted with heart failure for review in clinic.
10. Once competent, provide follow-up clinic as a specialist nurse in collaboration with Consultant Cardiologist, to include working toward and obtaining non-medical prescribing qualification.

Education
1. Act as role model and provide specialist clinical advice/education to colleagues.
2. Maintain own professional development and up-to-date knowledge of research relating to patients with HF.
3. Provide educational opportunities for staff and students wishing to improve their understanding of HF issues through teaching and placement visits.
4. Act as a resource on HF management for patients and their families, providing education to improve understanding of their condition and promote concordance with medication.
5. Contribute actively to the development of nursing practice within cardiology.
6. Identify and provide the specific education/support needs of patients, carers and staff within the arrhythmia service and respond to these needs whenever possible.
