The Cromer Group Practice have an exciting opportunity for an experienced diabetes specialist nurse to work within their team on the north Norfolk coast. This position is offered on a part-time basis. The successful post holder will work as a member of wider nursing team comprising of practice nurses and health care assistants, alongside secondary care colleagues and other agencies managing patients with diabetes. The main duties of this role are to provide comprehensive care management to our diabetic patients, providing them with clinical care, guidance and support with their diabetes. You will work to policies and protocols and strive to improve standards of care to the practices patient population. Main duties include: Liaise on regular basis with GPs, practice nurses and HCAs to assist in the development of the diabetes service. Contribute to the co-ordination of care between primary and secondary care diabetes services to ensure high quality of patient care and the continuity of services. Provide a pathway of diabetes nursing care for the patient, referring to secondary care, multidisciplinary teams and voluntary services as required. Maintain effective communication with patients, carers and professionals. Manage a complex caseload. Participate in formulating individualised patient treatment care programmes in conjunction with the patient, multidisciplinary team and initiating changes in management of care. Request and interpret diagnostic investigations and inform patients and GPs of results. Act as the patients advocate when relevant and educate on all the treatment options to enable a patient to make an informed choice. Ensure patients, carers and health care professionals are given verbal and written information regarding planned care. Maintain adequate patient records and ensure all relevant information is documented. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: Essential: Advanced clinical and diabetic knowledge. Current NMC registration with an active pin. Relevant nursing degree. Experience of running diabetes clinics in community or primary care. Diabetes qualification. General practice experience. Desirable: Prescribing qualification. Experience of SystmOne. We are made up of four GP partners, three salaried GP's, a practice nursing team of five nurses and two nurse practitioner's. We are a hybrid practice with a pharmacy attached to the surgery.
