Manage the surgery prescription line plus organization of the medicine managements task box. Be responsible for providing an accurate prescription service to patients, processing acute and repeat prescriptions in line with practice processes and KPIs. Receive and process medication requests both received in person and electronically, communicating effectively with patients and keeping them updated and informed on the progress of their requests. Facilitate effective communication between patients, members of the primary health care team, pharmacies and other associated healthcare agencies, either in person or via the telephone or email. Identify any changes required to synchronise patients medication when items may be out of repeat. Keep patients up to date and aware of the progress of their prescription request, using all communication methods available to ensure patients are informed Inform patients usual GPs of changes to medications and communicate with community pharmacies of changes to medication for patients who have set up repeat dispensing or receive MDS trays. Identify where medication reviews and other aspects of patient care are due and help organise this work. To maintain accurate contemporaneous records on all aspects of the care process and patient contact. To undertake general routine safety audits to support the clinical pharmacists and GPs with the medication review process. To ensure alerts are placed on patients notes relating to any prescribing issue To ensure all cancelled prescriptions are correctly audited on the EMIS system and destroyed accordingly Link/liaise with local strategic groups (e.g. ICCs/PCN) to ensure work is aligned with local health and wellbeing priorities. Awareness of, and compliance with, all relevant local/clinical policies and guidelines. Participate in data collection for audit purposes and be responsible for monitoring progress against key performance targets.
