Job summary 15 hours per week We are looking for an enthusiastic Senior Rehabilitation Assistant to join our Community Neuro Rehabilitation Service. The successful post holder will provide clinical support and planned rehabilitation for patients, supporting them in the community setting. This will include carrying out rehab sessions and ordering of equipment. The individual will also support the team with administrative tasks as needed. You will work closely with a team of Physiotherapists, Occupational Therapists, Specialist Nurses, Speech & Language Therapists, Psychologists, Rehab Consultants, other Rehab Assistants and Admin Support. This post requires community working and so access to a car for visits is essential. To provide comprehensive administrative support to the service. To effectively prioritise and manage own workload and oversee workload of team as necessary, whilst maintaining confidentiality and diplomacy at all times, both independently, or as part of a team. To be responsible for assessing patients, progressing and implementing programmes of care and reporting back to qualified staff. Providing planned rehabilitation care for clients who require regular support and intervention in the community, monitoring and modifying care plans as required. To work without direct supervision and show extensive use of initiative in prioritising and making decisions in relation to effectively supporting the smooth operation of the service. To carry out other duties as necessary to meet the changing needs of HIOWH NHS Trust, in accordance with management requirements and appropriate to banding. Desirable Neurological specific training e.g. equipment, gait re-education, handling, Neuro/Stroke Rehabilitation, LTC management, Palliative care, Postural management, Continence Care. Experience Essential Experience of regular assessment and treatment of patients presenting with problems requiring therapeutic intervention, some of a complex nature. Recent NHS experience of treating people with a range of stroke / neurological needs e.g. continence, mobility, balance problems, cognitive deficits, dysphagia. Relevant experience of working within a multidisciplinary team in either a hospital or community setting (some of which has been gained within a UK setting). Experience of working within a multi-professional team. Desirable Supervision and training of others.
