Overview
We are seeking a dedicated and compassionate Practice Nurse to join our healthcare team. The successful candidate will take a management role for the practice team of 4 nurses and 2 HCAs and phlebotomist, providing strategic planning, overseeing clinical governance, training and development.
Working in accordance with the NMC Code of Conduct, the post-holder will provide assessment, treatment, screening, health education services and advice to patients.
The post-holder will work with reference to Practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
The post-holder will have the required level of training and competence and will work within patient group directions where these are available. Although not a requirement of the post, if the nurse has prescribing qualifications they will use them appropriately.

Registered General Nurse
Qualifications: post qualification Diplomas in at least two of:
* Asthma
* COPD
* Diabetes
* Family Planning
* Travel Health
Experience
At least 4 years’ experience in practice nursing is essential.
Recent experience in Cervical Cytology is essential.
Recent experience in Travel Immunisation is essential.
Recent experience in Childhood Immunisation is essential.
Previous Management Experience is desirable.
Experience of Emis clinical system is desirable.
Duties and Responsibilities
Management Responsibilities
* Line manager for the nursing team.
* Enable the nursing team to work effectively to provide a high quality service to patients and to work effectively with other agencies.
* Overseeing the Clinical Governance of the nursing team and working with the GPs to ensure that we provide a first class service.
* Enable the Practice nursing team to apply and improve clinical governance and risk management systems.
* Monitor and advise on the effective use of physical and financial resources by the Practice nursing team.
* Lead and monitor the development and implementation of policies, standards and guidelines across the Practice nursing team.
* Analyse, interpret and report data on health and well-being, related needs and service use.
* Allocate, co-ordinate, monitor and assess the work of the nursing team and individuals.
* Recruit and select staff to meet the health and well-being needs of the Practice population.
* Supervise and assess staff performance and development.
* Provide clinical supervision and support to other members of the Practice nursing team and act as a clinical expert.
* Develop a working environment that promotes healthy, safe and effective work outcomes.
* Demonstrating compliance with CQC standards and preparation for inspection.
* Supporting with QOF targets.
Management of Chronic Diseases:
* Conduct annual reviews for cardio vascular and respiratory conditions, development of individual patient centred management plans at annual review; agreeing these as appropriate with the patient and other health professionals in accordance with current guidelines and with knowledge of QOF requirements and NICE guidelines.
* Identify and monitor the complications of chronic diseases.
* Advise re alteration of dosage where necessary (training if necessary).
* Educate, advise and support newly diagnosed patients and those with established chronic diseases.
Treatment Room Duties:
* Wound care management and dressings.
* Family planning advice, including IUD and pill checks.
* Injections.
* Removal of wound closure materials.
* Infection control.
* Assisting in covering for HCA duties when absent, for example phlebotomy, INR monitoring, BP and ECGs.
* Administration pertaining to Practice Nurse duties.
Immunisation (Adult and Child):
* Administering appropriate immunisation autonomously.
* Working with patients with more complex immunisation issues.
* Communicating with Child Health and D of H on immunisation queries and programmes.
* Communicating and updating Child Health Issues to the clinical team.
* Ordering of vaccines.
Travel Health:
* Individual risk assessment of each traveller's needs.
* Malarial advice and dispensing of appropriate prophylaxis.
* Administering vaccinations and providing guidance in accordance with guidelines.
* Identifying problems.
* Provision of specialist and evidence based support to the team.
Mental Health:
* Administering appropriate prescribed therapies and monitoring for side effects.
* Awareness of disease condition and reporting to GP of any concerns.
Risk Assessment:
* Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems).
Infection Control
* The post holder will be the Infection Control lead for the practice.
* Annual Infection Control Audit and Risk Assessment.
* Organise Annual staff Infection Control update.
* Liaising with the Contract Cleaners on Infection Control issues.
General
* Dealing with clinical queries from patients, practice staff and GPs.
Personal/Professional Development:
* In addition to maintaining continued education through attendance at any courses and/or study days necessary to ensure that NMC professional development requirements are met, the post-holder will participate in any training programme implemented by the Practice as part of this employment. Training courses are appraisal led and any need identified to be approved by the Practice Manager.
* Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
* Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
* Participation in annual basic life support and resuscitation update is mandatory.
Meetings:
* Monthly Management team meetings.
* Monthly Primary Health Care Team meetings.
* Practice Nurse Team meetings are held every 6 weeks, organised by the Nurse Team leader.
* Clinical Meetings with the GPs every two months.
* Protected Learning Time sessions are held for half a day, 6 times per year.

* In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
* In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, Practice staff and other healthcare workers. They may also have access to information relating to the Practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
