We are seeking to recruit a part-time 22.5 hours per week Community Practitioner to our Gateshead West Community Treatment team.
Via our ongoing service improvement models, we seek to recruit a suitably qualified person who is dynamic and motivated with a specific interest in the provision of top-quality secondary level mental health care within the community. The successful applicant will have appropriate knowledge and skills to assess, deliver, and coordinate treatment and adhere to local and national targets and NHS drivers.
As a band 6 Community Practitioner, you will also be required to offer supervision and support to other members of the team.
The successful applicant will possess good communication and interpersonal skills, be focused and well-organised, and have the ability to be flexible and responsive, demonstrating a positive, caring, and compassionate attitude to this role. Effective clinical skills, as well as the ability to stay motivated and the willingness to be creative in a stressful environment are a must.
Main Duties of the Job
1. Be responsible for the Assessment and Management of casework of service users who have substantial and complex needs.
2. Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, third sector partner agencies, and the public.
3. In collaboration with the Service User and Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plans as necessary.
4. Consider Carer needs and ensure Carer and family are involved in Care Planning where appropriate.
5. Deliver evidence-based Clinical and Psychologically based Interventions.
6. Work as a member of the Multi-disciplinary Team ensuring that high-quality individualised care is delivered, promoting recovery and wellbeing, and maximising independence.
7. Coordinate Treatment Planning & Interventions, Reviews, and Discharge Planning.
8. Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Team Manager.
9. Provide compassionate care based on empathy, kindness, respect, and dignity.
10. Incorporate a client-centred philosophy into nursing practice and client-centred collaborative care.
11. Work flexibly to meet the needs of Service Users across pathways, which may include working extended or flexible hours.

Person Specification
Education and Qualification
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
* Proven track record of post-registration clinical experience relevant to the post.
* Up-to-date knowledge/experience of relevant legislation.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
* Qualifications in Psychological Interventions.
* Experience of working within a community team.
Knowledge and Experience
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
* 12 to 24 months band 5 qualified nurse experience dependent upon service need.
* Understanding of difficult to manage/challenging behaviours.
Skills and Competencies
* Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
* Risk Assessment and Risk Management / Safety Planning skills.
* Evidence of a high level of professional, organisational, and leadership skills (self & others).
* Demonstrate effective written and verbal communication skills.
* Ability to adapt within a changing environment and work under pressure. Excellent interpersonal skills.
* Ability to use electronic recording and relevant IT equipment. Ability to work effectively with other professionals using a multi-agency approach. Ability to work autonomously and within a team.
* Clinical Leadership, Training & Induction of staff, and Delegation skills.
* Excellent time and case management skills. Good conflict resolution skills. Proven track record of acting as an effective advocate for service users and their carers.
* Demonstrate an understanding of effective & appropriate treatment options including a range of evidence-based clinical approaches as identified in N.I.C.E. Guidance.

£37,338 to £44,962 a year pro rata per annum.
