We are seeking an Allied Health Professional or Registered Nurse (Adult) to lead and manage the Hospice at Home Team, in order to deliver high quality Community End of Life Care. The successful candidate will support and develop the team of nurses who line manage a large team of healthcare assistants and ensure high quality and safe care for patients in the last weeks of life and their families. The Hospice at Home Team supports patients who are in the last weeks of their life and want to be at home. The team works closely with families, community nursing teams, and the wider hospice services to give the best care and support possible at this crucial time.
Main duties of the job
The Hospice at Home Team are experts in ensuring high quality End of Life care is delivered to patients and their families. They work independently and as part of the wider health care team. The Hospice at Home Operational Lead role provides clinical leadership, operational management, and professional support to the team. This includes staff recruitment and retention, skill mix, and team development.
1. Work with the Access and Community Nurse Specialist Operational Lead and Head of Community Nursing to develop the Community Nursing Strategy and set team objectives.
2. Manage, lead and develop the Hospice at Home Team to work efficiently and effectively in the delivery of high quality and progressive end of life care in line with the Hospice strategy and policies.
3. Contribute to the culture of leadership across the hospice.
4. Develop collaborative working relationships with other hospice teams and the wider community services to align services.
5. Be part of the Hospice at Home on Call/Out of hours rota with potential to join the Senior Clinical on Call rota.

Clinical practice
1. Oversight of the team-held clinical caseload of patients living in the community.

3. Evaluate how best to measure impact on patient care.
4. Take and analyse a clinical history in a relevant succinct and logical manner.
5. Independently assess the patients' needs, identify their priorities and work with the patient and family towards achieving them.
Person Specification
Experience
* Significant experience of leadership and management.
* Knowledge of clinical governance and evidence-based practice.
* Knowledge of issues related to the provision of palliative care in the community.
* Knowledge of safeguarding individuals who are at risk.
* Knowledge of relevant digital systems e.g. EMIS.
* Symptom management for patients at the end of life.
* Advance Care Planning.
* Experience of teaching.
Qualifications
* Current HCPC/NMC registration.
* Communication skills training, or willingness to undertake.
* Valid UK driving licence.
* Leadership training.
* Mentoring training.
* Independent prescribing.
* Physical assessment and clinical reasoning, or willingness to undertake.
