Main duties of the job
As a Clinical Lead Nurse, you will work closely with the Senior Clinical Lead Nurse, Registered Manager, and Head of Clinical Services to develop and implement individualised care plans, conduct holistic nursing assessments, and provide day-to-day oversight of regional care delivery teams. You will also manage and support care staff and field nurses, maintain accurate documentation, and collaborate with multidisciplinary teams to deliver outstanding care.

Are you an experienced Nurse (Band 6 or equivalent) with a passion for delivering complex care in the home? At Libertatem Healthcare Group, we’re seeking a dedicated Clinical Lead Nurse to join our team in the Midlands and make a meaningful difference in the lives of our clients.
You’ll work closely with our Senior Clinical Lead Nurse, Registered Manager, and Head of Clinical Services to provide leadership and deliver personalised, client-centred care. This is a unique opportunity to blend your clinical expertise with leadership and coaching responsibilities.
1. Develop and implement individualised care plans in collaboration with service users, families, and healthcare professionals.
2. Conduct holistic nursing assessments and monitor client outcomes, making proactive changes as needed.
3. Provide day-to-day oversight of regional care delivery teams, ensuring standards are met.
4. Manage and support care staff and field nurses through supervision, competency assessments, and appraisals.
5. Maintain accurate documentation and uphold compliance with statutory and contractual requirements.
6. Collaborate with multidisciplinary teams to deliver outstanding care.
7. Inspire and motivate care teams to embrace our company values and deliver exceptional care.
8. This role requires participation in an on-call rota approximately 1 in 6 weeks.
Qualifications
The successful candidate will be able to demonstrate the following:
* Ideally have community care experience, including expertise in spinal injuries, acquired brain injuries, and cerebral palsy.
* Hold a valid nursing registration with no restrictions.
* Be experienced in working with adults or paediatric clients (training provided where needed).
* Demonstrate strong IT skills and excellent communication abilities.
* Be passionate about delivering quality care and driving service improvements.
* Possess a full UK driving licence and flexibility to travel within the region.
In return we are offering:
* Salary range between £47,000 - £50,000 with a £3,500 car allowance.

Hours of Work:
Monday – Friday 9 am – 5:30 pm with flexibility for evenings and weekends to meet the needs of the client.
