Purpose 1. To undertake comprehensive physiotherapeutic assessments and to plan and execute treatment of patients with chronic respiratory problems in outpatient, clinic and community settings. These patients may have complex needs, both physical and emotional, including co-morbidities and may be at any stage of the disease process, including end stage. 2. To take an active role in the supervision and teaching of physiotherapists, nurses and other members of the multidisciplinary team regarding respiratory problems across the Trust. 3. To be responsible as an autonomous practitioner to undertake a significant specialist clinical workload. 4. To be an active member of the respiratory care team, including participation in supporting unplanned admission avoidance where appropriate, providing post discharge support, community clinics, home visits and Ambulatory Oxygen prescription. 5. To have responsibility for providing pulmonary rehabilitation sessions, both face to face and virtually for patients with respiratory conditions across NW CCG. 6. To contribute, once able, to the provision of an effective high quality Home Non-invasive Ventilation service 7. To participate actively in evaluating the quality and effectiveness of care delivered and make recommendations for changes. 8. To undertake evidence based projects, including recommendations for change in practice and to lead where appropriate in departmental evaluation/research. Primary duties and areas of responsibility: To deliver autonomous evidence-based patient care, achievable through direct patient contact To communicate effectively with all disciplines involved in a patients care both those within the organisation and those outside to ensure effective multidisciplinary working To be able to communicate complex and sensitive information to patients, carers and other staff including imparting unwelcome news relating to their rehabilitation potential To use persuasive and influencing skills to encourage patients to self-manage their conditions To be able to carry out spirometry and interpret spirometry results To work in conjunction with the multidisciplinary team across both primary and secondary care To be responsible for assessing patients respiratory needs referring them to other health care professionals as appropriate Provide autonomous advice and treatment recommendations with regards to patients specific respiratory and education needs in order to improve health outcomes. Promote health through empowering patients, offering appropriate health education, information and advice. To keep accurate, electronic and written documentation, care plans and reports To support the management of COPD exacerbations and other chronic respiratory conditions and support the discharge process from secondary care. To involve, support and educate patients and, as appropriate, their family and/or carers To treat all patients as individuals, respecting their privacy and dignity at all times To promote health and wellbeing to the patient and their family/carers To have an awareness of current national standards and guidelines relevant to Respiratory Disease and improve practice and health outcomes where appropriate.
