The Director for Community Transformation will lead a comprehensive and coherent programme of community transformation for the Staffordshire & Stoke-on-Trent Integrated Care System, working closely with local NHS, Local Authority, Social Care, primary care, voluntary and wider Public Sector community partners across health and care. They will be responsible for designing, developing, and implementing the Community Transformation programme - on time and to a high quality.
This is a key senior leadership role that will take full responsibility for the development and delivery of an effective and efficient transformation programme, in a complex and far-reaching portfolio, using high level influencing skills to enable and deliver successfully in a complex, multi-organisational and multi-professional networked model.
• Leading the development, design and delivery of the Community Transformation Programme – on time and to a high quality.
• Working with system partners including primary care – convening design workshops, co-designing new models, co-designing new specifications for services.
• Taking stock of current services models, investments and options and building on these to design a core offer for community services.
• Developing the commissioning processes to secure delivery of the core offer.
• Using population health information and intelligence to determine highest impact areas for change and reviewing evidence on interventions recommended to form part of the core and/or enhanced community services model of the future.
• Influencing at the highest level across multiple stakeholders, taking people with them, building ambition & finding solutions to how we shift demand from hospital-based settings to community-based settings.
• Building robust relationships with Local Authority commissioning teams and the Provider Collaborative Board.
• Putting in place a robust outcomes framework and benefits realisation oversight process, including monitoring delivery and taking action where there is slippage or deviation from planned outcomes/benefits.
• Holding the role of Portfolio Director role for End of Life and Frailty (ELF) – aligning the work of the portfolio, in particular the ICB’s commissioning responsibilities, under the overall Community Transformation Programme
The Staffordshire and Stoke-on-Trent NHS Integrated Care Board (ICB) is responsible for planning functions; managing the NHS budget and arranging the provision of health services in the geographical area.
Location Home based/hybrid working (most of posts). Travel on occasions to a Hub location with at least 1 days notice
New starters are expected to collect IT equipment, from offices in Stoke-on-Trent, and if based at home are required to have appropriate desk/chair/ relevant equipment in compliance with DSE regulations
Posts at Band 8c and above, will take part in the On-call rota, flexible over 7 days, supporting the System Control Centre, with full training provided.

Strategic Planning and Transformation
• Direct the delivery of the Community Transformation programme holding corporate level responsibility on behalf of the Chief Transformation Officer. This will include managing the complex interdependencies across the programmes and ensure each programme is aligned.
• Design and lead service transformation along clinical pathways in line with the System MTP.
• Co-ordinate work to take stock of transformation progress against current services models, investments and options and building on these to design a core offer for community services - including assessment of:
- change maturity
- investment and impact
- learning on implementation and on stakeholder partnerships and setting out a set of recommendations
• Work with the Health and Care Senate to ensure clinical engagement in transformation, contributing as appropriate to the development and implementation of the emerging clinical pathways and standards.
• Work with the ICB Primary Care Team and primary care providers to ensure full engagement of primary care providers in the design and delivery of future models of community services, which include services in which primary care providers may be taking the lead
• Support staff, user and carer involvement in partnership with local Population Health Leads in the planning and design of inclusive services and co-produce innovative new models using recognised transformation, programme management and quality improvement processes and informed by intelligence and research
• Ensure appropriately embedded service evaluation and encourage the involvement of staff in research, training and development relevant to the service.
• Work in a community-orientated way, taking account of diverse communities and the political context, to promote social inclusion, equity of access, experience, outcome, recovery and personalised support for service users.
• Develop and deliver high quality IT based reports and communication materials around the community transformation programme.
• Use population and public health information and intelligence to and insight to inform the emerging models of care.
• Develop culturally sensitive and competent services acceptable to a diverse population and to ensure fair access to services irrespective of gender, sexuality, religion, culture, race or disability.
Programme Design, Delivery and Management
• Oversee delivery of all work streams, maintaining quality and working collaboratively with local leaders and other programme teams.
• Monitor, analyse and evaluate innovative ways of working that improve existing and future working practices across the programmes.
• Develop and deliver detailed implementation and resources plans to ensure successful delivery of the programme plans for service users, staff and services.
• Provide robust and detailed programme management leadership to drive delivery of the programme.
• Develop robust plans to support scale up, sustainability and spread
Relationships and Communication
• Lead the overall community services transformation programme, convening partners across the system in the design, commissioning and delivery of this work which forms a major component of the Medium-Term Plan.
• Take responsibility for ensuring effective matrix working across ICB directorates and between system partners to improve performance and outcomes for patients.
• Act as the lead senior manager for day-to-day communication and engagement with the Local Authorities in relation to community services transformation
Financial stewardship and budget management
· Overall responsible for the multiple budgets within the ELF and Community Transformation portfolios.
· Responsible for allocating and budget setting relating to the resources of the teams, according to the key priorities and identifying financial solutions where required. This may include the procurement of external resources.
