Here at Darent Valley Hospital we are looking for 2 The Stroke Nurse Specialist to work on our newly developed Acute stroke unit, where they will contribute to the delivery of a high quality service for adults who have had a stroke.
The successful candidate will be required to provide patient-centred clinical care, encompassing the skills of assessment, examination, investigation, diagnosis and treatment, within an agreed scope of practice, throughout the Acute Stroke/TIA/Thrombolysis Service, leading to the safe referral and discharge of patients across the hospital and community.
As senior nurse within the stroke service, you will be integral in forming the model of delivery for the Service that will achieve a cohesive, effective and seamless pathway for the stroke patient journey.
A key element of this role will involve the education, training, advising and support for a variety of staff and stakeholders involved in delivering stroke care within the Dartford & Gravesham NHS Trust area.
Main duties of the job
1. Manage a cohort of patients referred for specialist intervention, care and support within Stroke services, being a point of contact for assessment, information and advice for the patient, family and carers and all staff involved in the care of stroke patients.
2. Undertake comprehensive patient assessment and screening, including those with complex and multi-pathological presentations of stroke.
3. In conjunction with the Ward Manager and stroke clinical team, identify which is the most suitable patient to be transferred from the Hyper Acute Stroke Unit to the Acute stroke unit to accommodate acute stroke emergencies.
4. Participate in ward rounds and case conferences as required.
5. Deliver safe care within a challenging and changing clinical environment, using equipment and resources safely and as appropriate.
