Main Duties and Responsibilities:
1. Independently assess physical, psychological, spiritual, cultural, religious, social, legal, and ethical issues affecting the patient and carer.
2. Proactively engage in the development of services.
3. Plan and deliver appropriate care in consultation with the multi-professional team.
4. Facilitate communication and co-operation with other health care multidisciplinary teams and with both statutory and voluntary agencies.
5. Be aware of changing policies, practices, and innovations within nursing and palliative care to promote evidence-based clinical intervention, care, and support ongoing developments at KEMP.
6. Support and supervise Student Placements, Care Services volunteers, and support workers across the hospice and in the community.
7. Provide flexibility in role and approach, dependent upon patient and organisational needs.
8. Act as infection control link.
9. Work in partnership and support the Care Services Team, plan, implement, and evaluate care from first assessment to discharge or death.
10. Ensure patients' views are taken into account when planning care by establishing their care needs and preferences both on admission and continually thereafter, including identifying advance care planning needs.
11. Demonstrate practice that is evidence-based and up to date.
12. Use professional knowledge and experience to influence decision-making to improve outcomes for patients and the team.
13. Keep accurate clinical records in accordance with KEMP's policy and procedures, ensuring all data is recorded accurately on the clinical management software system.
14. Participate in collaborative decision-making with all those responsible for delivering care within the Care Services team to ensure that patients' and carers' wishes and preferences are represented in every aspect of their care.
15. Perform clinical procedures competently according to KEMP's policies, procedures, and practice; provide nursing and personal care as required.
16. Maintain accurate patient electronic records.
17. Negotiate and advocate on behalf of patients and carers and recognize the need to refer to and liaise with other agencies and professionals.
18. Maintain patient confidentiality, advocacy, and respect patients' rights and wishes regarding treatment and care.
19. Participate in reflective practice/clinical supervision.
20. Work in accordance with the NMC Code of Professional Conduct.
21. Assist the Care Services Manager in meeting and maintaining the standards set by the Care Quality Commission.
22. Contribute to the supervision and support of the Care Services volunteers.
23. Take appropriate action to record, monitor, and report complaints, accidents, and untoward incidents, in line with hospice policy.
24. Welcome all visitors to KEMP, promoting good patient care and both internal and external relations.
25. Ensure safe custody and administration of drugs in accordance with Hospice policy.
26. Assist with the orientation and induction of new Hospice staff.
27. Be willing to prepare for and lead morning handover and MDT meetings.
28. Participate in the ongoing development of our care services in day hospice and the community in conjunction with colleagues.
29. Participate in and promote education and training for staff, volunteers, and visiting students both formally and informally.
30. Maintain personal professional development pertinent to current role and linking own development needs with KEMP's appraisal and objective setting process.
31. Promote the use of evidence-based practice.
32. Comply with mandatory training and other training mutually agreed with the Head of Care.
33. Participate in audit evaluation and implement changes to improve patient care.
34. Contribute to research-based projects.

36. Support and encourage harmonious internal and external working relationships.
37. Make a positive contribution to fundraising and raising the profile of the Hospice.
38. Provide support and advice to colleagues within KEMP within your area of expertise by working closely with the Multi-disciplinary Team within KEMP.
39. Always behave in a professional manner, reflecting and maintaining KEMP values as set out in the KEMP way.
