The Community Mental Health Assessment Team offers a screening and assessment service to adults (aged under 65) in the Bradford District and Craven region. Those referred to our team are those felt to be potentially experiencing complex or severe mental health difficulties and who may need specialist support.
The team comprises Community Mental Health Nurses, Social Workers, a Nurse Prescriber, and Psychological Therapists.
We are closely linked with Community Mental Health Teams in the Bradford District and Craven region. When mental health needs are identified that may need support from one of these teams, we are able to plan and transfer care to them.
We carefully consider each referral. Where appropriate, we can give advice to Primary Care or help people to access other forms of support available in the local community.
