Job Description
Service Care Solutions has a new and exciting opportunity which has become available working as a Transitional Support Worker with Tameside Council.
Tameside Council is currently looking for someone who is happy to work 37 hours per week.
Key Responsibilities:
* Establish face-to-face contact with young people and engage them with the service.
* Conduct needs assessments and develop tailored plans to help young people achieve their aspirations.
* Provide support for accessing education, training, employment, and community-based activities.
* Liaise with external agencies to secure necessary support for young people, including housing, health services, and therapy.
* Deliver group sessions and positive activities that promote personal growth and community involvement.
* Uphold safeguarding principles and respond appropriately to any concerns.
* Advocate for young people's voices, ensuring their views shape the services provided.
* Maintain accurate records and manage caseloads effectively, adhering to departmental procedures.
* Contribute to the ongoing development of the Staying Close Service.
About You:
We're looking for a compassionate, motivated individual who is passionate about supporting young people.
Essential Qualifications:
* NVQ Level 3 in Care of Children and Young People, Adult Social Care, or equivalent.
Essential Skills and Experience:
* Proven experience working with cared-for children and families.
* Familiarity with Child Protection Procedures and relevant legislation for Care Leavers.
* Strong IT, administrative, and organizational skills.
* Ability to build meaningful relationships with young people and their families.
* Knowledge of child development, attachment theory, and effective assessment techniques.
* A commitment to anti-discriminatory and equal opportunity practices.
Desirable Skills and Experience:
* Experience supporting families with substance abuse or domestic violence concerns.
* Knowledge of parenting capacity assessments and evidence-based practices.
* Understanding of care planning and court processes.
* Full driving licence and access to a car for business use.
