An exciting opportunity has arisen to join our Intensive Services team, supporting the delivery of person-centred care services across East Renfrewshire and enabling individuals to remain safe and supported within their own homes.
You will have a good understanding of health and social care, the positive impact of timely discharge from acute settings, and the value of outcome-focused supports to help people return to and stay in their communities. You will also recognize the importance of building and maintaining effective relationships with other health and care professionals and agencies.
Your responsibilities will include assessing and reviewing service provision for service users, supporting referral pathways for access to care at home from hospital and community settings, and assisting individuals in continuing to live independently by arranging care services and providing appropriate advice and support.
This post is considered Regulated Work with Protected Adults under the Protection of Vulnerable Groups (Scotland) Act 2007. It is an offence to apply if you are barred from working with protected adults. Additionally, if you have lived or worked outside the UK in the past 10 years, there may be a requirement for an overseas police check.
