Job summary
As a Specialist Practitioner you will work across the four pillarsof advanced practice. You will lead and deliver Nurse led respiratory clinicsfor patients in our Brownlow Central practice. The focus will be Asthmamanagement within our large student population as well as review of patientswith other respiratory conditions such as COPD, Bronchiectasis and PulmonaryFibrosis. You will also provide support to our homeless population withbi-monthly clinics.
Whilst working within your own sphere of competence you will workautonomously to proactively target high risk patients to provide an assessmentof their needs and provide an appropriate management plan. The post holder willhave a high level of expertise and experience in this field and the ability tomanage complex cases including first contact assessment, examination andmanagement of patients presenting with acute respiratory problems.
You will provide support and mentorship to our less experiencedPractice nurses whilst also supporting in education to the wider team.
The post holder will also be expected to work with the practice nursing team, contributing to data gathering as needed to achieve the Practice fulfilment of the Asthma and COPD targets within the Quality Outcomes Framework.

Primary key responsibilities
Deliver Nurse led respiratory clinics to support the PracticeNurse team
In line with targets within the Quality Outcome Framework and NICEguidelines, the post holder will review patients with asthma and COPD usingrelevant EMIS clinical templates to ensure assessments are completed and codedto meet QOF requirements.
Use of non- medical prescribing to review medications includingmaintenance therapy and acute self- management plans.
Assessment of inhaler technique and optimization of inhalertherapy, giving consideration to greener respiratory care such as low carboninhalers, over usage, overprescribing.
Provision of a written personalised asthma/COPD action plan.
Review of patients admitted to an emergency care setting with anacute asthma attack or exacerbation of COPD within 2 working days of discharge.
Review of patients admitted to hospital with symptoms ofbreathlessness related to any other underlying respiratory problem or LRTI.
Ensure patients coded COPD have had spirometry performed toconfirm diagnosis.
Ability to interpret spirometry testing as required
First contact assessment, examination and management of patientspresenting with acute respiratory problems including potential Covid 19symptoms.
Review patients diagnosed/suspected with long Covid
Administration of relevant health prevention vaccinations
Provide home visits to ensure our housebound respiratory patientsreceive a timely review.
Request and interpret relevant blood tests and investigations.
Build links with and liaise with secondary care and communityservices
Work within national guidelines, policies and procedures as wellas local protocols supporting the development of these in house as needed toensure the patient received the highest quality evidence based care.
Maintain effective communication with patients, carers, relativesand members of the multidisciplinary team ensuring information is sharedappropriately.
