An exciting opportunity to join a forward-thinking and specialist multi-disciplinary team to work with patients with chronic pain within the Frimley ICS. We are looking for someone who has an interest in chronic pain and is passionate about empowering patients to develop the skills and strategies to self-manage their condition.
Ideally you will have experience of working alongside patients with chronic pain or long-term conditions and using movement based therapy and breathwork as tools for improving quality of life.
Our multi-disciplinary team consists of Pain Consultants, Clinical Nurse Specialist, Clinical Health Psychologists and Advanced Practice Physiotherapists that work closely together to deliver comprehensive assessments and formulate treatment plans. The biopsychosocial model underpins our practice, and we all strive to work in a psychologically informed way. This is supported by ACT, trauma informed care, CBT and mindfulness.CPD is supported throughout the team and this is achieved through in-service training and external courses. As part of the team you will have access to a CPD funding and links with a practice development for Allied Health Professionals.
Main duties of the job
* To take a leading role in the advanced assessment, clinical diagnosis and treatment of patients with Chronic Pain.
* To use highly specialist, physiotherapy knowledge and skills to manage a complex caseload as an autonomous practitioner.
* To ensure a thorough in-service training programme is provided to all members of the team to maintain clinical competency.
* To act as a specialist resource for the multi-disciplinary team and Physiotherapists within FHFT.
* To work with the MDT to develop, monitor and evaluate the Chronic Pain service, enhancing pathways in line with national guidelines
* To lead in appropriate research and audit in the specific clinical area.
* To support the Advance Practice Physiotherapist and deputise for them as appropriate.
* To be physiotherapy co-ordinator and link for clinical specialism, ensuring excellent overall management of patients

Job responsibilities
To be responsible for the efficient, effective and safe physiotherapy management of all patients within speciality.
To receive referrals, undertake full assessment and plan individual, specialised, evidence-based, treatment programmes for patients, this will require sound clinical reasoning and a variety of highly specialist skills
To use recognised outcome measures to evaluate the effect of physiotherapy interventions and ensure the treatment programmes are progressing appropriately.
To prioritise work efficiently taking into account clinical and service priorities, using effective organisational skills and highly specialist knowledge to underpin decisions.
To be professionally and legally responsible and accountable for all aspects of own work, including the management of clinical risk, knowledge of indications and precautions of chosen techniques in the line with national and trust clinical guidelines, protocols and pathways where they exist.
To advise, guide and teach physiotherapy students, in the management of patients and their related problems whilst they are on a placement with Frimley Health NHS Foundation Trust.
To develop profession specific practices and local guidelines and keep abreast of new physiotherapy techniques in relation to the management of patients within speciality.
To establish good relationships with patients promoting and empowering a life long model of self care and appropriate activity to maintain good health.

* Demonstrate up to date knowledge of best clinical practice in relevant speciality
