Based at the Acute Stroke Team on ward B11 at Peterborough City Hospital, you will join a supportive and ambitious team in delivering high-quality assessment and evaluated care required to support each patient in their care pathway and improve the patient experience.
B11 is divided into 2 areas: 12-bed monitored HASU for patients admitted for specialist acute care, 22-bed acuter and rehabilitation stroke beds and 1 dedicated assessment trolley for thrombolysis/acute stroke or TIA patients.
You will have a passion for delivering high standards of care, commitment and motivation to help drive our services forward.
We offer a 24-hour thrombolysis service and link with regional centres for mechanical thrombectomy. We also provide a service for TIA patients and are actively involved in Stroke Research and Stroke national data collection (SSNAP)
We have Stroke Coordinators available 24/7, Stroke Consultant cover 7 days a week in HASU, 5 days a week in ASU and specialist registrars.

As a senior member of staff, you will act as a clinical nurse leader, taking responsibility for the professional development of yourself and a group of nurses. Your responsibilities will include ward coordinating in charge of a shift and senior cover in HASU or ASU day and night to support the team, sharing your knowledge and clinical skills.
Throughout your journey, you will work in close liaison with the Ward Manager, other Deputy Sisters/Deputy Charge Nurses, the Stroke Coordinator, Matrons, the Practice Development Team (PDT) and the Multidisciplinary Team (MDT).
You will ensure the patients are supported along their pathway liaising with MDT, other agencies, and family / NOK ensuring good communication and care is provided in a timely way.
You will also have the opportunity to complete the stroke coordinator competencies and follow the patient from ED to HASU.

Leadership and Clinical Practice
• Maintain clinical credibility to act as a role model and take a lead role in the patient’s journey.
• Deputise for the Ward/Department Manager when required.
• Act as shift coordinator to facilitate effective patient care pathways and ensure that patient care remains a priority in inter-professional practice and that timely referrals to the MDT and discharge planning take place.
• Act as clinical leader and participate in the clinical management of the ward/department ensuring practice is safe and Trust policies are followed.
Education
• Develop own clinical practice and skills through training and education.
• Once competent as an assessor, train and competency assess others.
• Participate in Ward/departmental in-service training programmes to help develop staff.
• Act as a clinical resource, mentor and where appropriate sign-off mentor to learners.
Safety
• Be aware of the safeguarding agenda. Role model best practice in line with safeguarding policy, ensuring protection and identification of vulnerable patients.
• To report all adverse events and near misses and take appropriate action. Follow up investigations and where required work with the ward manager to share lessons learnt for practice.
• Participate in a wide range of local and trust-wide patient safety initiatives such as falls and pressure ulcers.
Effectiveness
• Working with the ward team and wider MDT, develop productive ways of working to deliver the best care to patients. Identify new ways of working to improve the patient experience.
• Foster an environment where the practice is research/evidence-based and undertake and participate in audits to ensure this is effective. (Safety audit, matron’s balanced scorecard, stroke audit etc.)
Patient experience
• Act as a patient advocate through the application of ethical, legal, and professional knowledge and skills as required.
• Increased partnership working with the patient, ensuring that quality of patient care is delivered, monitored, and sustained.
