Community Learning Disability Nurse / Health Facilitator

We are seeking two highly motivated, enthusiastic and passionate Learning Disability Nurses to join the DTV ALD Physical Health Team as Band 5 Community Nurses / Health Facilitation Nurses. One will be based within Teesside and one in Durham.
The team provides specialist advice and support to both people with a learning disability and to health services to reduce the health inequalities and premature mortality that people with a learning disability face.
This role is a full-time permanent position within the team, and the working hours are Monday - Friday 9am-5pm. As part of the role, you are also required to participate in the Adult Learning Disability Nurse on Call (Monday-Sunday 5pm-9am).
This post is only open to those candidates who already have their NMC registration in place and have successfully completed the preceptorship programme. Please note: Newly qualified applicants or those due to qualify will be recruited via a separate process.
Main Duties of the Job
The successful candidate will work with health colleagues within the wider Learning Disability Community Teams, Local Authority colleagues and in liaison/partnership with primary and secondary care services, using a Person-Centred Approach to enable and support adults with a learning disability and their carers to promote the health needs of people with a learning disability and enable access to wider health services.

Person Specification
Qualifications
* Current professional registration with Nursing & Midwifery Council RNMH or RNLD
* To be recognised as a practice assessor/mentor or work towards this in an agreed timescale
* Trained Clinical Supervisor (within 6 month probationary period)
* Evidence of Continuing Professional Development
Experience
* Working with people with learning disabilities/mental ill health
* Working in a person-centred therapeutic organisation
* Working collaboratively with service users and their families/carers
* Working in a multi-disciplinary team
* Working in partnership with other agencies
Knowledge
* Evidence-based knowledge of best practice in caring for patients in the designated field
* Understanding of relevant legislation (e.g., Mental Health Act, Mental Capacity Act)
* Understanding of Safeguarding and its application in practice
* Clinical Risk Assessment and Management and its application in practice
* Clinical Governance and its application in practice
* Confident working knowledge of clinical medicines management, including administration of injections
