A fantastic opportunity has become available in the Discharge Lounge. We are looking for a highly motivated, enthusiastic Nurse to join our team. The Discharge Lounge has become an essential part of the Trust, supporting capacity and flow through the emergency and planned care pathways by releasing inpatient bed capacity earlier in the day, and enhancing the patient journey by providing a safe environment for patients to wait while the final arrangements for discharge/transfer are being made.
Main duties of the job
* Proactively co-ordinate discharges within the discharge lounge.
* Ensure the Discharge Lounge is utilised on a daily basis to create flow, reporting to Matron and Bed management where failure occurs.
* Maintain custody of controlled drugs, checking and maintaining ward stock of drugs in accordance with local policy and legislation.
* Monitor effective nursing documentation which is kept on all nursing activities.
* Ensure safe custody of medical/nursing records and reports.
* Ensure effective communication channels are maintained with multi-disciplinary teams.
* Provide support, education and knowledge on all aspects of discharge planning to the staff within the Discharge lounge.
* Ensure an effective learning environment is established and maintained for all trainee staff on the ward.
* Develop, maintain, and participate in programmes for Preceptorship and Clinical Supervision.
* Participate in the setting and monitoring of standards in relation to patient care, ensuring qualitative and quantitative aspects of care are evaluated and addressing any non-compliance to standards.
* Be instrumental in developing practice in the light of research.
Person specification
* Registered Nurse with specialist training or experience of discharge management in an acute hospital setting.
* Experience of working in Nurse led discharge.
* Excellent written and verbal communication skills with the ability to converse in a positive manner with all levels of professional colleagues, carers, and patients.
* Experience in ward management.
* Evidence of leadership skills.
* Computer literate with good computing skills and ability to use programmes such as Word and Excel competently.
