An exciting opportunity has arisen for a Mental Health Nurse Band 6 to work within the HPFT Integrated Care Team within the Hertfordshire area. Our Integrated Care Mental Health team (ICT) are a team of professionals who specialize in working with people who have long term physical health conditions, including frailty, which may be impacting on their mental health. Our team aims to support service users to continue to live independently in their own home.
This is a unique position for someone who enjoys teamwork, challenges and variety in their day to day work. You will be working collaboratively with other health colleagues with service users who have complex frailty needs that are impacting on their mental wellbeing. This position is based in the Hertfordshire area, supporting our service users with mental health and wellbeing, including nursing and residential homes.
Main duties of the job
You will be expected to:
1. Visit service users who are identified under the frailty pathway, in their place of residence, to complete a holistic assessment of their physical and mental health needs.
2. Take responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care and management of a defined caseload, including liaison with other agencies and, where appropriate, the supervision, development and teaching of staff and/or students.
3. Work in partnership with other healthcare professionals and trusts. Integrated Care Teams aim to make healthcare more preventative, this service supports service users with multiple long term or complex conditions to remain at home rather than going into hospital or residential care. Referrals are accepted from various sources such as primary care, ambulance service, A&E and secondary care.
4. Hold a caseload of service users, ensuring the care plan is met at all times.
5. Be part of a wider supportive and holistic multidisciplinary team of mental health nurses, mental health OT's and Psychology team.

The post holder will:
1. Be responsible for the full assessment of care needs and risk assessment of service users presenting with a wide variety of clinical conditions. Following this, the post holder will be responsible for the development, implementation and evaluation of programs of care, without direct supervision (though clinical supervision is implemented as per policy to support you).
2. To be named primary worker and act accordingly when applying care.
3. To manage oneself in a professional manner, responding to requests, enquiries etc in a timely fashion.
4. To respond to any crisis that a service user may advise of, with the aim of promoting independent living in the community.
5. Work with the philosophy to support service users to remain in their own home.
6. Administer and/or supervise medications as required. To have a sound knowledge of medications and their desired and undesired effects.
7. Provide education, advice and guidance to service users and their careers and families.
8. Provide a range of clinical interventions and treatments appropriate to the individuals' needs.
9. Participate fully in providing quality care in line with local and national guidelines.
10. Act as a role model in providing a service for people with mental health problems.

Person Specification
KNOWLEDGE, TRAINING AND EXPERIENCE
Essential
* Degree, diploma or equivalent in registered Nursing (Mental Health Nursing).
* Registration with Nursing and Midwifery Council.
* Mentorship Preparation Course.
* Two years post registration experience, with at least one year post registration experience in Older Adult Mental and or Frailty health care.
Desirable
* Pre or Post registration training/short courses in mental health.
* Community and Inpatient nursing experience.
* AMHP/NMP qualification.
* Physical healthcare training.
* Experience of working with clients with Delirium.
* Tier 2 Dementia training.
* Experience of working in primary care.
AREAS OF EXPERIENCE AND KNOWLEDGE
Essential
* Knowledge of legislation related to mental health (e.g. Mental Health Act, Mental Capacity Act, Care Act, Care Programme Approach etc).
* Good knowledge of Microsoft Word, PowerPoint, and Excel.
