Locum GP Pharmacist
Opportunity available for a GP Pharmacist to work within a GP Practice, based in Greater Manchester.
Responsibilities include:
1. Undertaking structured clinical medication reviews with patients and producing recommendations on prescribing and monitoring.
2. Reconciling medicines following discharge from hospitals, intermediate care, and into care homes, including identifying and rectifying unexplained changes.
3. Working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.
4. Setting up systems to ensure continuity of medicine supply to high-risk groups of patients.
5. Producing and implementing a practice repeat prescribing policy.
6. Managing the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions.
7. Reviewing medicines reaching review dates and flagging those needing a review.
8. Ensuring that patients have appropriate monitoring tests in place when required.
9. Identifying cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches.
The hours for this role are 20 Hours Per Week.
