Team Leader - Palliative & End of Life Care
An exciting opportunity has arisen to join the Trust as the Macmillan Palliative and End of Life Care Team Leader in Wexham Park Hospital. This post encompasses a combination of clinical, managerial and leadership responsibilities, coordinating and delivering the vision of the 7 Day Hospital Palliative Care service.
The successful applicant will work as part of a well-established Palliative Care and EoLC multi-disciplinary team providing specialist advice and support to patients, their relatives, and members of the healthcare teams within the hospital and community. Applications are invited from a Nursing & Midwifery professional background who have worked at a senior level in palliative care. You should also have a Degree in Nursing, a recognised palliative care qualification, advanced physical assessment skills, be a non-medical prescriber, and possess excellent communication skills.
The postholder will be proactive in leading service redesign in palliative and end of life care within the hospital to make services responsive to changing needs. You will work closely with the Trust Lead for EOLC and the Palliative Care MDT.
Main duties of the job
1. Provide leadership and support for the Palliative and End of life Care team at Wexham Palliative care team.
2. Act in a consultancy role as an advanced clinical practitioner in palliative care, offering specialist advice, guidance, and supervision to clinical staff, patients, and carers.
3. Act as an expert in palliative and end of life care and advise on care for patients with advanced malignant and non-malignant disease from the time of diagnosis and throughout the trajectory of their illness.
4. Lead developments in practice consistent with the patient care priorities set out nationally and locally. Develop new roles and redesign services in line with best practice and evidence-based care.

Job responsibilities
1. Lead and manage the palliative and end of life care team daily, ensuring the team provides an excellent service to the Trust through evidence-based practice, education, policy, and guideline development.
2. Lead clinical care by providing expert assessment, planning, evaluation, and support skills in aspects of palliative and end of life care.
3. Maintain a high profile in clinical practice, promoting clinical excellence, initiating learning opportunities, and promoting an effective learning environment.
Person Specification
Qualifications
* Degree in Nursing
* Masters degree or commitment to work towards completion
* Qualification in Palliative Care/Oncology/Frailty
* Registered Nurse with current NMC registration
* Counselling qualification
Experience
* Recent experience in managing and leading a team
* Ability to work at pace, establish effective working relationships, and develop strong teamwork
* Ability to manage multiple and conflicting priorities and meet specific deadlines
* Proven leadership and management skills
* Experience of leading patient involvement/patient feedback initiatives
* Involvement in clinical audit and/or research
Knowledge
* Ability to organise and prioritise own workload
* Knowledge and experience of management models, implementing change and/or new clinical practices
* Knowledge of clinical governance and risk management
Skills and Aptitude
