Job summary An exciting opportunity is available to contribute to the provision of Respiratory and TB Services in Luton community services to meet the needs of people with respiratory conditions, including COPD ,Asthma, ILD, Bronchiectasis and TB who require community support. This role will include: To provide specialist nursing advice for both primary and secondary care and work to prevent TB by vaccination, education and the monitoring of patients receiving care and treatment. In conjunction with UK Health Security Agency Staff, primary and secondary care this post implements preventative and infection control measures according to agreed protocols. To support the delivery of education, support and training for service user, community nursing teams and primary care. To work with colleagues to provide high quality, evidence based, integrated services for adults and older people To work towards increasing community access to support a reduction in health inequalities and increase quality of life for patients with respiratory conditions Clinical responsibility for the assessment, planning and co-ordination of care for a caseload of patients, to avoid unnecessary hospital admissions and prevention of acute exacerbations. Main duties of the job 1. To support the vision, purpose and objectives of the Community Respiratory and TB Services 2. To support the Service manager/Lead Nurse to ensure services achieve their quality, performance, productivity, financial and contracting requirements. 3.
