We are hiring! Taking on this exciting challenge you will be responsible for direct delivery of therapeutic occupational and physiotherapy activities for people with a diagnosis of organic mental health illness and associated problems related to the Aging process, in particular, Dementia, Challenging Behaviours’ and Physical Disabilities.
You will carry out all relevant forms of care in support of and supervised/directed by a Registered Occupational Therapist/Physiotherapist and will support the therapy and nursing teams in the implementation and delivery of high standards of patient care promoting patient wellbeing following a ‘Person-Centred Approach’.
You will working under the guidance and direction of registered staff to assess the social, psychological needs of patients by undertaking designated assessments in activities of daily living, cognition and mood.
This is primarily within the ward but could be in community settings (home) as appropriate.
The perfect match for our team will undertake programmes of personal care duties for patients, consistent with their agreed plan of care; whilst maintaining/promoting independence, privacy and dignity
Undertake programmes of care to maximise/ maintain patient’s independence.
To work as a member of a multi-disciplinary Team on the patients, behalf, assisting with co-ordinated care on the patient’s admittance, care period and planned discharge
Plan and organise own routine day to day task and activities
Participate/initiate/ prepare social and therapeutic interventions with patients, to achieve a planned programme of care and a stimulating environment
Record clinical observations or test results personally undertaken; and collect samples for investigate purposes
Receive and give reports to the therapy / nursing team regarding patient progress, condition and treatment
Work in an environment with distressing/emotional circumstances
Follow nationally accepted guidelines as directed when dealing with aggressive/abusive situations and attend the Managing Violence and Aggression courses as mandated whilst supporting patients, staff members, carers and the public in stressful situations
Use judgement to identify challenging situations and defuse and distract patients to promote the safety of patients, other staff members, carers or other visitors to the ward.
