MAIN DUTIES AND RESPONSIBILITIES:
1. To provide a social work service to adults living in the Bailiwick of Guernsey.
2. To carry out assessments using the agreed standardised format.
3. To participate in co-ordinating services for people wishing to remain within their own homes.
4. To offer support to people who may be moving into care.
5. To work in a flexible way, adapting the service to meet the changing needs of service users from a wide range of social and cultural backgrounds.
6. To work in an identified social work team within the Integrated Adult Community Care Service.
7. To form relationships with service users to formulate an accurate assessment of complex psychosocial needs of service users, their families and service providers and assess for services in line with local policy.
8. To contribute towards a holistic assessment within the multi-professional specialist team, recognising that diversity, values and human rights may influence the decisions of people who use the service.
9. To provide complex psychosocial support for service users with terminal illness/life threatening illnesses and their families, advocating for service users, carers, and families around complex emotional & practical needs.
10. To provide specialist social work advice within the Adult Community Health & Social Care Team.
KEY CRITERIA:
MINIMUM QUALIFICATIONS:
Recognised Social Work qualification E.g. Diploma/Degree in Social Work or CQSW FULL
QUALIFICATIONS: Evidence of post graduate study in relevant area
ESSENTIAL 1. Proven experience of working in integrated adult community services.
11. To possess a full and valid driving licence.
12. To be enthusiastic, with good interpersonal skills and experienced in working in multidisciplinary teams, where the contribution of the Social Worker role is promoted.
13. To be flexible in terms of adaptability to new situations, i.e. working practices / locale / developing legislation / co-ordinating structures.
14. Proven ability to manage a dynamic, and often complex caseload with experience of undertaking a range of assessments, care planning and completing safeguarding reports, taking the lead in safeguarding investigations.
