NHS Norfolk and Waveney Integrated Care Board
To clinically assess patients for NHS Continuing Healthcare eligibility in patients' homes, care homes, and hospitals.
To follow the process laid out in the National Framework for NHS Continuing Healthcare and participate in MDT eligibility recommendations.
Ensure assessments are robust and complete new referrals for NHS Continuing Healthcare within 28 days as per the National Framework for Continuing Healthcare.
Main duties of the job
1. Carry out joint assessments with Social Service colleagues to identify individuals whose care needs meet the criteria for NHS Continuing Healthcare.
2. Proactively manage a delegated caseload and demonstrate timely forward planning and the ability to organize oneself.
3. Complete relevant specific core reviews/assessments for NHS funded individuals e.g. Deprivation of Liberty/Liberty Protection Safeguards.
4. Deal with people with complex needs and challenging behavior, including those approaching end stage of life.
5. Handle difficult family situations and dynamics, requiring a high level of competent effort, objectivity, and reflective practice.

Person Specification
Experience
* Educated to degree level or equivalent level of experience of working at a similar level.
* Working knowledge of safeguarding adults and the Mental Capacity Act.
* Clear communicator with report writing skills.
* Independent thinker.
* 2 years post-qualification experience, including caring for complex patients.
* Community nursing experience, independent sector, or hospital experience.
* Further training evidenced.
* Good understanding of Health and Social Care environments.
* Workforce development knowledge and experience.
