Due to retirement, we have an exciting opportunity for a Practice Nurse to join our multi-disciplinary team working across the week between the hours of 0830 and 1830, and ideally one Saturday morning in 3 as part of a rota (we are open one Saturday a month). A qualification in diabetes is desirable.

Main duties of the job
The post holder will focus on supporting patients to be healthy, monitoring long-term conditions, health prevention and screening activities in line with the objectives of Bramblehaies Surgery. They will work collaboratively with the whole general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and be a valued member of the nursing team.
The post holder would be able to run a full Practice Nurse Clinic. This would include but not be limited to providing general nurse treatment room cover and Chronic Disease Management.
The post holder will demonstrate safe clinical decision making, assessment, and expert care.
As a Practice Nurse, the applicant will be required to maintain accurate patient records, ensure accurate completion of all necessary documentation associated with patient health care, and contribute to the preparation of any practice development plans.
The successful applicant will always promote a professional approach by attire and attitude to patients and staff, both in and out of the practice. They will also be required to maintain their own continuous professional development and ensure their PDP is kept up-to-date, in line with the requirements of the practice.

Bramblehaies Surgery, in Cullompton, is a semi-rural training practice in the heart of Devon serving a population of 7594 patients, providing high standards of care (CQC rating of Good) from a partner owned modern, purpose-built building.
Our team currently consists of four GP partners, a salaried GP, Practice Manager, Patient Services Manager, Data Quality & Systems Administrator, Clinical Pharmacist and Pharmacy Technician, Nurse Practitioner, a team of three Practice Nurses headed up by a Lead Nurse, three HCAs, a General Practice Assistant, a committed admin/reception team and our much-needed housekeeping team. Our patient list is growing so no doubt our staff team will continue to grow to meet the needs of the population we provide medical care to.
Job responsibilities
The following are the core responsibilities of the Practice Nurse. There may be on occasion a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
1. To develop, implement and embed health promotion and well-being programmes
2. To implement and evaluate individual treatment plans for chronic disease patients
3. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health
4. To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
5. To provide wound care (Doppler etc.) to patients
6. To provide travel medicine advisory services
7. To request pathology services as necessary
8. To process pathology results as required
9. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
10. To maintain accurate clinical records in conjunction with extant legislation
11. To maintain chronic disease registers
12. To develop, implement and embed well woman clinics
13. To develop, implement and embed well man clinics
14. To chaperone patients where necessary
15. To assist GPs with minor surgery, joint injections and coils when required
16. To prioritise health issues and intervene appropriately
17. To support the team in dealing with clinical emergencies
18. To recognise, assess and refer patients presenting with mental health needs
19. To implement vaccination programmes for adults and children
20. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
21. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
22. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
23. To support the clinical team with all safeguarding matters in accordance with local and national policies
24. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
25. To deliver opportunistic health promotion where appropriate
