We have a very excitingopportunity within the Wakefield Intensive Home Based Treatment Team as we havea vacancy for a band 3 Mental Health Support Worker, this is a rare occurrencefor our team as these roles do not become available very often. The team currently consists of SocialWorkers, Nurses, Medical Practitioners, Psychologists, Trauma InformedPersonality Disorder Practitioner and a Non Medical Prescriber, Dual DiagnosisPractitioner, as well as Health Care Support workers and Admin staff.
The team operates within thewhole system of adult mental health services, and is the single referral pointfor people in crisis, offering assessment and the provision of intensivehome-based treatment for those for whom hospital admission would otherwise benecessary.
The service operates 24 hours aday 365 days a year, the health care support workers work a shift system whichrequires you to work 3 long day shifts per week ( hours each). Thisrequires working both autonomously and as part of the MDT.

Main duties of the job
The successful candidate will becommitted to team working, have excellent practice skills and the ability towork flexibly and creatively in order to meet the needs of service users andtheir carers. You will also be required to work autonomously with the supportof the MDT.
The successful candidate will becarrying out home visits on an individual basis as well as supporting theircolleagues with patient contacts that may require more than one practitioner.There would be an expectation for documentation to be carried out to a highstandard with the opportunity to gain knowledge on the wider services availablein the Wakefield area.
We offer regular supervisionwhich includes team supervision as well as individual supervision, personaldevelopment is actively encouraged through interagency training and any other ContinuedProfessional Development you may find of benefit to your practice

The post holder will -
1. under the supervision and direction of registered health care professionals and associate practitioner, within a delegated framework for agreed tasks, seek support as and when necessary, in the delivery of psychological and physical care to a defined group of people.
2. as part of a healthcare team, provide the highest standards of safe and compassionate planned care to people that maximises their recovery and rehabilitation potential.
3. develop and maintain meaningful therapeutic relationships with people. Adopt a polite, positive and empathetic approach, communicating effectively and informatively.
4. demonstrate an understanding of the care and treatment programmes of a designated group of service users to enable you to instruct, advise and guide service users, family and carers. To make the appropriate adjustments independently within own level of competence and be able to communicate any changes to service users and registered health care professionals.
5. be required to concentrate for periods of time, depending upon the care intervention and care plan eg, following the observation policy, taking, recording of physical observations etc, and responding to issues of concern appropriately. This may include areas of work requiring physical effort, with regard to risk, health & safety.

24. Care Certificate or required to achieve within 12 weeks of commencement
