Job Description Senior PCN Clinical Pharmacist Accountable and reporting to: Location: Lead Clinical Pharmacist Rushcliffe PCN THE PURPOSE OF THE ROLE General Practice is at the heart of the Health and Social Care System, and it is critical to have thriving, sustainable general practices that meet local peoples health needs. The Senior Clinical Pharmacist is instrumental in supporting their own practices clinically and developing and mentoring the more junior pharmacy workforce within Rushcliffe PCN. This is a key role, intended to support the delivery and development of the pharmacist role in primary care and ensure that we continue to provide high quality, accessible and responsive services to both practices and patients. KEY RESPONSIBILITIES Works with the Lead Clinical Pharmacist and GP Clinical Lead for pharmacy to deliver the strategy for clinical pharmacy within the PCN. Delivers effective clinical and managerial leadership to allocated members of the pharmacy team. Supports Lead Clinical Pharmacist with the training and clinical supervision of the wider PCN pharmacy team Demonstrates understanding and implementation of appropriate governance. Demonstrates understanding of, and contributes to, the workplace vision. Demonstrates ability to improve quality within limitations of service. Acts within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians in general practice. Works as part of a multi-disciplinary team in a patient facing role. Takes responsibility for areas of chronic disease management within the practice and undertakes clinical medication reviews to proactively manage patients with complex polypharmacy. Provides primary support to general practice staff with regards to prescription and medication queries. (helps support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice) Provides clinical leadership on medicines optimisation and quality improvement and manages some aspects of the quality and outcomes framework and enhanced services. Ensures that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. Summary of primary duties of the senior PCN pharmacist 1. Line manager for PCN clinical pharmacy team Day to day line management of clinical pharmacy team who have differing competencies and abilities. Complete monthly 1-1 reviews with all staff for whom you have line management responsibilities. Responsible for 3-month probation review, 6-month probation review and annual appraisals for all staff for whom you have line management responsibilities. Responsible for 6 monthly prescribing audits for all Independent Prescribers for whom you have line management responsibilities Be the link between PCN leadership and management team (including PCN Clinical Pharmacy lead) and PCN pharmacy team. Create development plan for all staff that you have line management responsibility for. Follow professional and organisational policies/procedures relating to performance management Follow all HR processes correctly Demonstrate understanding of the implications of national priorities for the team and/or service Demonstrate ability to extend boundaries of service delivery within the team 2. Development/training and supervision of PCN clinical pharmacy team. Support with recruitment and induction. Ensure the team are receiving regular supervision. Provide training to support the development of individuals, as well as to the wider multi-disciplinary team (MDT). Provide support for supervisees with CPPE pathway Willing to be DPP for pharmacists within the team who are studying for their IP qualification and/or undergraduate pharmacists. Understands and demonstrates the characteristics of a role model to members in the team and/or service Demonstrates understanding of the mentorship process 3. Service development Work in partnership with PCN leadership for the implementation and delivery of the medicines related elements of the Network Contract DES and other significant contracts / frameworks. Work in partnership with PCN leadership to deliver digital and operational transformation. Work closely with stakeholders to lead on reviewing, developing, and providing high quality pharmacy services within the PCN. Look at service improvement and development through lens of reducing local health inequalities. 4. Lead quality improvement for the PCN pharmacy service 5. Representation of service Attend relevant meetings related to their speciality, role, and experience.
