Sunderland Care and Support are seeking to recruit Care Co-ordinator (Level 1) into our Woodland View and 3 Fenwick Close services, based within the Physical and Complex division. We are looking for candidates who can deliver exceptional standards of person-centred care and support to those with a high level of physical and mental health support needs.

Your role will involve providing support to customers promoting and developing their life, health, and wellbeing. Contacting customers, families and several MDT, obtaining information and developing care plans to enable the team to provide a person-centred support. Some day-to-day tasks may include:
Helping with preparation of meals & drinks. Support with domestic household tasks.
Administering & recording medication
Support with money management.
Encouraging community trips and facilitating customers participation
Support with household & personal shopping.
Support with meeting friends & family.
Support with high standards of nutrition.
Adhere to health & safety legislation.
Cleanliness & intimate personal hygiene
Opportunities for employment or education
Provide emotional support.
Active support techniques & positive behaviour support techniques to increase skill level & achieve set outcomes (training will be provided)

Salary £25,584 - £27,269
Designated rota pattern including daytime, evenings, weekends, and Bank Holidays
