The Children’s Rapid Response Service have an exciting opportunity as it continues to expand the team. The Children’s Rapid Response Service works collaboratively with the Community Children’s Nursing & Palliative Care Team: It is the team’s primary aim to prevent and reduce hospital attendances, admissions and length of stay for children already known to the Community Children’s Nursing & Palliative Team.
The emphasis of this service is to improve the quality of life for children and families with complex healthcare needs, offering them greater choice around their chosen place of care.
Are you an enthusiastic, dynamic, flexible and motivated experienced Nurse or Nurse Practitioner, with a strong knowledge of Children’s Nursing? Clinical expertise and good communication skills are required as well as excellent time management and prioritisation skills which are essential for this role. The service is open from 07:30 – 22:00 across 7 days and there is flexibility to work short shifts or long days.
We are looking for experienced children’s nurses that have extensive experience in assessment with children with complex health care needs.
You must have the ability to prioritise your workload using advanced assessment skills with the ability to provide a symptom management plan of care to children/young people during times of acute deterioration or illness.
The role requires undertaking clinical assessments of a sick child or young person and emergency enteral feeding management to prevent hospital admissions.

Professional
1. Work in accordance with NMC Code of Professional Practice and Scope of Professional Practice. Professionally and legally accountable and responsible for all activities.
2. Work within the community (e.g. within client’s own home) as a lone practitioner managing situations of risk with telephone support. Seek support and advice with cases from more experienced colleagues when faced with matters outside sphere of responsibility, knowledge or skills.
3. To provide a calm and supportive approach to the management and delivery of sensitive and complex information, providing both practical and emotional support, utilising flexibility and empathy in difficult situations.
4. Use effective communication techniques communicate to the multidisciplinary team to ensure the child and family referred to Children’s Rapid Response receive the best possible care.
5. Undertake risk assessments prior to service delivery within the home setting to ensure staff safety.
6. Work in partnership with children, families and carers to provide optimum individual outcome acknowledging children and family’s personal preferences, spiritual and religious beliefs.
7. Formulate individualised management and treatment plans using sound knowledge and evidence based practice. Plans should be in response to sound clinical reasoning and include expected signs of deterioration.
8. To provide timely, effective symptom management, resulting from a comprehensive history taking utilising independent non medical prescribing, GP’s or hospital team, completing the symptom flow charts to monitor the child’s changing condition/deterioration.
9. Provide and promote strategies/detailed plans of care to parents and carers empowering care that will prevent inappropriate admissions and reduce length of hospital stay.
10. Ensure assessment, planning and delivery of holistic care is of the highest standard utilising an agreed nursing model.
11. Liaise and work in collaboration with acute services, primary healthcare team and other agencies when planning and implementing programmes of care facilitating early discharge and preventing admissions.
12. Advise the Team Leader, Clinical Lead or Service Manager of any circumstances which give cause for concern in relation to patient care and/or any concerns raised by the primary healthcare team or partner agencies.
13. Maintain clear, concise and contemporaneous records adhering to BCHC record keeping policies.
14. Provide a flexible service to accommodate the changing needs of children and families.
Management
1. In the absence of the Team Leader/Clinical Lead provide leadership to ensure the effective service delivery.
2. Responsible for accepting referrals and discharging patients from own caseload.
3. Responsible for reassessing children, ensuring regular formal review of the caseload in conjunction with the Team Leader, ensuring these are documented and evidenced as per record keeping guidelines.
4. Ensure standards are maintained with both clinical quality and governance being central to the function and delivery of the service.
5. Accountable for advice and guidance to patients, families, carers and other team members.
6. Act as an effective role model for other team members, students and professionals.
7. Participate in research and audit relevant to the service in conjunction with the Team Leader/ Clinical Lead.
8. Appropriately manage the service resources e.g. equipment, staffing, supplies in conjunction with the Team Leader/Service Manager.
9. Develop policies, procedures and guidelines in conjunction with the wider multidisciplinary team.
10. Ensuring staff maintain organisational standards in relation to record keeping.
11. Undertake risk assessment for own clinical caseload, action planning effectively in response to risk assessment. Ensure effective communication to line manager relating any circumstances which give rise for concern.
12. Manage informal complaints from service users raising any recurrent themes or concerns with the Clinical Lead and Service Manager and assist with formal investigations as requested.
13. Demonstrate critical thinking, problem solving and decision making skills.
14. Work positively with colleagues to maintain effective working relationships.
15. Accurately record and maintain client records daily using electronic and paper systems e.g RiO.
Education and Development
1. To maintain and update knowledge and skills in relation to the post.
2. To identify and undertake specific training to maintain skills in line with NMC Scope of Practice and development needs of the service.
3. Participate in the teaching of students and other staff within the clinical and university environment.
4. Participate in the teaching and mentorship of staff as required, ensuring they have the knowledge and skills to fulfil their roles.
5. Maintain own professional development in line with post-registration ongoing education and practice, Revalidation.
6. Act as a link Nurse and take clinical lead in areas of expertise. Attend courses and meetings as required to fulfil the link role as delegated by the Team Leader.
7. Participate in reflective practice/clinical supervision, critical incident analysis and debriefings.
Management and Leadership Responsibilities
1. Develop and empower all members of your team to perform to high standards and innovate.
2. Ensure supportive staff management arrangements are in place and carry out personal development reviews for direct reports. Ensure all staff in your team/s have annual PDR’s resulting in specific objectives and effective personal development plans in line with the Knowledge and Skills Outline Framework.
