The client is a 43-year-old female living independently with her dog. This is an excellent opportunity for a Nurse who is passionate about making a real difference in complex community care, and who values flexibility and relationship-based care delivery. Role Responsibilities: Provide clinical oversight and support for diabetes and associated health needs. Deliver high-quality, person-centred nursing care that promotes independence and emotional wellbeing. Collaborate effectively with the existing HCA team to ensure a cohesive approach to care. Administer medication, monitor clinical needs, and manage risk proactively. Build rapport with the client, offering continuity and stability within her home environment.
