About the MH team
We are happy to announce an exciting opportunity for a secondment Social Worker post in the Mental Health Social Work Team (South). This is a great opportunity to be part of the multi-dynamic and thriving work we do. The Mental Health Social Work teams undertake a wide range of statutory duties and functions relating to safeguarding adults, hospital discharge planning, advocacy, care management for adults in the community and residential settings, undertaking CHC assessments and many more.
About the role
1. You will be responsible for assessing and care planning of adults who have need for care and support arising from mental health & OR physical disability.
2. Working collaboratively with health partners from (BEH) Barnet, Enfield and Haringey mental health trust as well as other health and social care colleagues as part of multi-agency approach.
3. Responsible for undertaking a range of statutory care and support functions under the Care Act and assessing eligible mental health needs - Section 117 aftercare under the Mental Health Act 1983.
4. Being part of a supportive and co-productive team under a united & vibrant service area.
5. Conducting person-centred and strengths-based assessments to meet identified social care needs.
6. To plan relevant responses to meet individual needs in collaboration with relatives, carers, advocates and representatives, and in consultation with health and other agencies.
7. Responsible for safeguarding adults at risk which is a fundamental element implementing principles embedded in all procedures, practices, professional advice, decisions and support.
8. Conducting individual complex assessments and safeguarding enquiries to establish clear, relevant, proportionate, and appropriate options for the individual to meet their identified outcomes.
About you
1. Hold relevant Social Work degree qualification and registered as a Social Worker with the Social Work England.
2. Understanding and experience in working with vulnerable adults.
3. Relevant experience in operating effectively as a practising Social Worker.
4. Understanding and experience in operating with a comprehensive assessment framework including self-assessment.
5. Experience in preparing clear and accurate correspondence, reports and other written documentation.
6. Record of high performance in successfully delivering a customer-focused service to demanding targets and objectives.
7. Experience in actively and effectively participating in cross-functional networks and local groups.
