South Tyneside Older Adult’s Community Treatment Team
We are seeking to recruit a skilled and motivated community practitioner to join our experienced and effective team.
The successful applicant will have an interest in working with older adults and possess the appropriate knowledge and skills to support this client group. We are looking for someone who is passionate about working with older adults, who can work effectively as part of a team while also being able to work autonomously in this demanding role.
Please note this role will be based in South Tyneside.
As a community practitioner, you will provide evidence-based interventions, risk assessment, and management to a broad range of people with functional and organic mental health needs. You will be responsible for the assessment, planning, implementation, and evaluation of care needs and manage a defined caseload. You will be supported by an approachable and experienced leadership team committed to clinical supervision and professional development.
You must strive to deliver high-quality, safe patient care and be a good role model in reflecting the trust's core values while offering leadership and supervision to junior members of staff.
There will be opportunities to work within the different functions of the team such as care home liaison, step up, and diagnostic pathway, as well as augmenting with other services to support patient care.
The team serves the population of South Tyneside and is an established team with a friendly and supportive culture.
Our current working hours are 9 am-5 pm Monday to Friday (excluding bank holidays) with occasional needs to work out of hours to meet individual patient/service needs, between 8 am and 8 pm.
