We are a busy practice located in central Halifax with a practice population of 17,000. We are looking for someone to join us who is proud of their professional achievements and has the ability to deliver exceptional patient care. You will be keen to contribute to this already successful practice as an Advanced Nurse/Clinical Practitioner with a sound knowledge of both acute & routine illness.
Main duties of the job
We are looking for the right candidate to join our team. Working pattern to be discussed at interview stage. We are happy to consider part-time or full time up to 37.5 hours per week.
The successful candidate will be an independent prescriber; will have experience of delivering telephone consultations, directing care appropriately and managing complete episodes of care independently. We would like someone with post-registration experience who ideally is working in a primary or urgent care environment. We would expect you to hold, or be working towards, a MSc in Advanced Clinical Practice.

Job responsibilities
Key Responsibilities, in particular but not limited to:
* Act as an expert practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles.
* Work as an autonomous nurse/clinical practitioner providing high quality and timely patient-centered care.
* Be an independent prescriber.
* Have experience of delivering telephone consultations, directing care appropriately, and managing complete episodes of care independently.
* Undertake consultations with patients as an autonomous practitioner and, using own clinical judgement, diagnose, treat, refer and/or discharge patients.
* Recommend, instigate, and explain appropriate invasive and non-invasive diagnostic tests, investigations, and interpret findings and reports.
Person Specification
Experience
* You will have experience of delivering a telephone triage service, directing care appropriately, and managing complete episodes of care independently.
* You will have a willingness to travel and hold a current valid driving licence.
* Management of minor illnesses or ailments.
* Proven ability in the safe management of acute and ongoing illness in patients presenting with undifferentiated diagnoses.
* Able to work autonomously, without immediate support and supervision.
* Proven ability to evaluate the safety and effectiveness of own clinical practice.
* Two years ideally working in a primary or urgent care environment.
Qualifications
* You should hold, or be working towards, an MSc in Advanced Clinical Practice.
* Masters Degree in Advanced Clinical Practice.
* Current Registered Nurse (NMC/RCN), Registered Paramedic (HCPC), or Registered Physiotherapist (HCPC).
* Minor Illness Diploma or equivalent level of qualification/experience.
* Evidence of Continuing Professional Development (CPD).
* Valid UK Work permit (if needed).
* Independent non-medical prescriber.
* Additional qualifications consistent with MSc Pathway (Advanced Clinical Practice, Clinical Patient Assessment, Research & Audit).
