Advanced Nurse Practitioner (GP practice- 8 H * 3 D/ Week)
* To assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways).
* Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.
* Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
* Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
* Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
* To develop new and innovative concepts, models, methods and practices to deliver new and improved primary care services to meet the needs of the practice and PCN population.
* To provide education and training to other staff and students.
Main duties of the job
Assess patients presenting with acute illness or for review of chronic disease conditions.
Diagnose, plan, implement and evaluate treatment/interventions and care for patients.
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
Fully participate in the clinical administration within the practice/PCN, signing prescriptions, reviewing medication, reviewing patient letters/discharges, respond to queries as required.
Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies.
Refer patients as appropriate to other members of the PCN multidisciplinary team, secondary care and to other statutory and voluntary organisations including referral for x-ray.
Undertakes and interprets a range of diagnostic tests and routine clinical procedures e.g., cytology, venepuncture, blood pressure monitoring etc.
Makes critical judgements of the highest order to satisfy the expectations and demands of the job.

Hounslow Family Practice is a partnership of two GPs, providing care for 6,100 patients in Hounslow. Our exceptional clinical team includes 2 GP Partners, 2 Salaried GPs, a Clinical Pharmacist, 1 Practice Nurse, 1 Physician Associate, and 2 Health Care Assistants. This clinical team is supported by an administrative staff, which includes a Practice Manager, an Administration team, and a Reception team, all dedicated to serving our patient population.

CLINICAL PRACTICE
The postholder will:
Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice.

Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.
Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.
Exercise professional judgement to manage risk appropriately, especially where there may be complex and unpredictable events and supporting teams to do likewise to ensure safety of individuals, families, and carers.
Work collaboratively with an appropriate range of multi-agency and inter-professional resources, developing, maintaining, and evaluating links to manage risk and issues across organisations and settings.
Evidence the underpinning subject-specific competencies i.e. knowledge, skills and behaviours relevant to the role setting and scope, and demonstrate application of the capabilities to these, in an approach that is appropriate to the individual role, setting and scope.
