Job overview
The Swansea Bay Acute Clinical Team is a dynamic Nurse Practitioner led Service, supported by Consultant Geriatricians, which provides hospital-based treatment in the patient's home. Currently, there are two hubs: one based at Bonymaen Clinic covering the Swansea geographical area and one based at Cimla Health and Social Care Centre covering Neath Port Talbot.
Both hubs cover vast geographical areas and in order to fulfill this role, the successful applicant must hold a full UK driving licence and have access to a vehicle for work purposes.
The Role of the Band 5 Registered Nurse includes the ability to recognize and assess acutely unwell adult patients, to communicate precise details of clinical presentation with the Advanced Clinical Practitioner in order to commence a treatment plan.
The Role includes:
1. Intravenous cannulation and phlebotomy skills
2. Safe administration of intravenous/subcutaneous therapies monitoring effectiveness
3. Anaphylaxis and cardio-respiratory arrest recognition
4. 12 Lead ECG recordings, bladder scanning, male and female urinary catheterization, collection of microbiology samples, maintaining high standards of documentation and safe keeping of patient records
5. Recognition of vulnerable adults and how to escalate concerns
6. Mentoring students while on placement to the Service
7. IT Skills
8. Attendance and contributions at daily Board rounds at Base
Main duties of the job
Advanced Practitioner led Service supported by Geriatricians which includes assessing acutely unwell adults in the community environment by providing hospital-based treatments in the patients' home.
The successful candidate must hold a full UK driving licence at the time of application due to the volume of driving required.
The successful applicant will require experience caring for adults in an acute setting and be able to recognize the deteriorating patient.
