Social Worker (36hrs) Based at Southport Hospital

As a qualified Social Worker in a busy hospital Social Work team, you will be responsible for fulfilling the Local Authorities statutory duties under the Care Act. This includes assessing individuals and arranging practical and social support for vulnerable people to facilitate a safe and timely discharge from hospital.
You will manage a defined case load delivering high quality assessments and support plans that address the needs of vulnerable people through the commissioning of individual packages of care, whilst upholding the excellent reputation of the Council.
