This role will require experience, as well as excellent clinical / managerial leadership. Advanced communication skills and the ability to act as an excellent role model are fundamental aspects of this role. Acting as patient advocates when needed, an understanding of mental capacity and DOLS is an important part of this job.
The successful candidate will be expected to work alongside the existing Nurse Practitioner / Team Lead, as well as be part of the wider MDT ensuring high quality rehabilitation care is provided. A keen interest in rehabilitation is vital for this role.
The right candidate will be enthusiastic, dynamic and able to work under pressure. Awareness of computer systems and electronic patient records is advantageous. The Newcastle upon Tyne Hospitals NHS Foundation Trust defines a nurse practitioner as a qualified nurse where the primary purpose of the role is to perform specific technological procedures, e.g. endoscopy, ultrasound in a defined patient group and/or advanced patient assessment and clinical decision making in a broad patient group.
To deliver a quality of service matching clinical need to available resources within the focus of specialist nursing practice: to ensure that the patient is the central focus of a streamlined patient journey ensuring optimal quality of care. Working as a nurse practitioner, provide a prompt response to calls from colleagues, prioritising importance and clinical need. Manage your own caseload to include assessment, investigation, diagnosis and treatment within defined parameters and training.
Provide immediate care management of patients including communication of plans to patients and significant others. Review and refer to the appropriate person, e.g. AHP, resident medical staff, specialist registrar or critical care outreach team, liaising with the Patient Services Coordinator where necessary.
Communicate within the multidisciplinary team, ward/departmental staff and/or out of hours team. Provide specialist education and training to departmental nursing staff, nursing and medical students.
