An exciting opportunity for a Band 6 Physiotherapist to join our Integrated Community Team. You will work closely alongside our nursing and frailty teams and wider Primary Care Colleagues to prevent hospital admissions, support patients being discharged from hospital, palliative care and promote self-management for patients with long term conditions.
We are currently working to develop and expand the delivery of UCR (Urgent community response) model of care whilst continuing to deliver rehabilitation to adults living in the area who have long-term or complex health needs.
We are looking for applicants who are dynamic, keen to develop their skills and who enjoy working as part of a team. The successful candidate will need to have good communication skills, demonstrate good time management and prioritisation skills. The post holder will participate in the development, training and support of the multi professional team and students. Applicants must be a registered physiotherapist and have full access to a vehicle for work and possession of a full driving license is essential. The post holder will also need to be willing to work flexibly to support our 7 day service model.
Main duties of the job
To provide advanced clinical and professional Physiotherapy to the Community Therapy Team, and to provide support to the Integrated Community Care team leaders in ensuring that the team contributes to the implementation of Hampshire and Isle of Wight Healthcare Clinical Governance Strategies.
Undertake Physiotherapy assessments of patients with diverse and complex conditions, providing a clinical diagnosis (where appropriate), development, and delivery of individualised treatment programmes.
To support the case management process by providing expert clinical advice and therapy to patients with a variety of complex conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission to secondary care.
As a supervisor, delegate tasks to assistant practitioners and junior therapists relevant to their skill level and competence.

Person Specification
Qualifications
* Degree/Diploma in Physiotherapy
* Registration with HCPC
* Training in the use of standardised physiotherapy assessment tools or models of practice.
* Membership of the Chartered Society of Physiotherapy (CSP).
* Membership of CSP -Special Interest Group -relevant to career.
* Practice educator training
Experience
* Experience as a qualified Physiotherapist.
* Relevant specialist/clinical experience and competence in chosen field including:
* Complex therapy/care co-ordination/manual handling
* Ability to demonstrate a holistic approach to addressing diverse patient needs.
* Understanding of role of PT as a complex intervention
* Experience of multidisciplinary working
* Experience of supervising staff and/or students
* Ability to work autonomously and set own priorities.
* Experience in applying a range of assessment tools and a comprehensive knowledge of physiotherapy clinical approaches.
* Able to communicate complex condition related information to patients, carers and colleagues.
* Partnership working with individuals, groups, communities and agencies.
* Experience in carrying out clinical audit and risk assessment, outcome measurement.
* Excellent interpersonal skills and ability to build and maintain relationships with other health and social care professionals, patients, carers and the public.
* Excellent written and verbal communication skills
* Strong IT Skills -- including excel and power-point.
* Good Presentation skills / public speaking.
* Ability to meet deadlines and work under pressure.
* Ability to adapt to the demands of a constantly changing environment.
